Linda Delaney writes for The Nursing Library. Her books come from one gap that turns up on every unit: the job runs on documentation, and almost nobody is taught how to do it well. Not the forms, which anyone can learn in a week, but the thinking underneath them. What to write. What to leave out. How to build a note that still holds up when a stranger reads it two years later, someone who remembers neither the shift nor you. She writes for the nurse who is already good at the job and was simply never shown the ropes on this one part of it. That is why the rules that matter most arrive with a worked example, and why there are forty-two of them: a rule you cannot see inside a real note is a rule you will not reach for at 0300. Her books are built around those worked examples, real notes shown before and after, and they are checked against named standards: The Joint Commission "Do Not Use" list, the ISMP error-prone abbreviation list, the federal Conditions of Participation. Where a rule is a law, an accreditor's standard, or simply what most units do, the book says which. Where no national rule exists, it says that too.
